What Is Two-Phase Orthodontic Treatment?

Two-phase orthodontic treatment is an approach designed for children who may benefit from orthodontic care at two different stages of development.

The first phase takes place while your child still has a combination of baby and permanent teeth. Treatment at this stage focuses primarily on jaw development, available space, and the relationship between the upper and lower jaws.

Next Steps

After Phase One, your child enters a resting period while additional permanent teeth erupt. Once the permanent teeth are in place, Phase 2 can begin. This stage focuses on positioning each tooth properly and creating a balanced bite.

Not every child needs two phases of treatment. Dr. Nicholas or Dr. Thomas will evaluate your child’s individual needs and recommend early treatment only when there is a clear reason to begin before all permanent teeth have erupted.

How Does Two-Phase Treatment Work?

Two-phase treatment is carefully timed around your child’s growth. Each stage has a different purpose, but both work toward the same goal: creating a healthy smile with teeth and jaws that function well together.

Phase 1

Phase 1 treatment helps guide jaw development to better accommodate your child’s permanent teeth. It may also improve the way the upper and lower jaws fit together as your child grows.

At this stage, treatment can address developing problems that may become more difficult to correct later. Depending on your child’s needs, Phase 1 may involve orthodontic appliances, limited braces, or another treatment designed to guide growth and create appropriate space.

Once we have achieved the goals of Phase 1, active treatment stops and the resting period begins.

The Resting Period

The resting period gives your child’s remaining permanent teeth time to develop and erupt naturally.

A successful first phase helps create room for these teeth to follow healthier eruption paths. During this period, we’ll continue monitoring your child’s smile at scheduled visits, watching how the permanent teeth come in and how the jaws continue to develop.

These check-ins help us determine the right timing and plan for Phase 2.

Phase 2

Phase 2 usually begins once most or all of your child’s permanent teeth have erupted.

The goal during this stage is to place each tooth in its best position and create a bite that works properly. Phase 2 typically involves full upper and lower braces, although the exact treatment plan will depend on your child’s needs.

After active treatment is complete, your child will wear retainers. Retainers help maintain the new tooth positions and protect the smile you and your child worked hard to achieve.

Who Is a Candidate for Two-Phase Treatment?

Two-phase treatment is not automatically necessary for every child who needs orthodontic care. In many cases, a single phase of treatment during the teen years is enough.

However, early treatment may be recommended when we identify concerns involving jaw growth, bite development, crowding, or the way permanent teeth are erupting.

Your child may benefit from two-phase treatment if they have:

  • Significant crowding or limited room for permanent teeth
  • Upper and lower jaws that are not developing in balance
  • Crossbites, underbites, or other developing bite concerns
  • Permanent teeth that appear likely to erupt in unhealthy positions
